                           One2One Device Usage Agreement Terms

Students must abide by the District's Responsible Use Principles (RUP). Student acknowledges that any issued device is the property of Borden-Henryville School Corporation and will abide by the following terms:

• Student MUST bring their device to class fully charged and ready to work each day 

• If the student DOES NOT bring the device to school, an email will be sent to the parent/guardian/student.

• Student MUST make the device available to any school administrator or teacher for inspection of any messages or files sent or received on their Borden-Henryville School Corporation ­issued device. 

• Student MUST report to building administration or teacher any incidents of inappropriate communications sent in any form using their Borden-Henryville School Corporation ­­issued device.

• Student MUST not deface the device in any way. NO Stickers, NO Drawing, NO Etching, NO Writing, etc... on the 

device or protective cover.

• Student MUST return device to the school main office BEFORE withdrawing/graduating from Borden-Henryville School Corporation. If the student does not return the device, legal action may be necessary to recover the cost of the device. 

Warranty and Repairs

• Manufacturer Warranty - Device is covered by a standard manufacturer’s warranty for any manufacturer defects. This does not cover abuse or accidental damage.

• Accidental damage would only be covered by the (Optional) Accidental Damage Coverage listed below. 

• Parent/Guardian will be sent an email or receive a phone call when a device is determined to be abused or damaged.

• Accidental Damage Coverage - (Optional) Accidental Damage Coverage is available for purchase to provide coverage for repair costs for damage occurring accidentally. (This does not cover abuse!)

• This annual coverage can be purchased each year for: $30 

• Payment for Accidental Damage Coverage must be received within 2 weeks after the student receives the device.

• Claim Year is from July 1, 2025 – June 30, 2026. This coverage resets in July every year.

• 1 Claim Per Year - Student is allowed to claim Accidental Damage 1 time per Claim Year 

• Without Accidental Damage Coverage - Full cost of repair or replacement for the device will be the responsibility of the parent/guardian/student.

• Intentional Damage/Gross Neglect - Replacement or Repair Cost is the responsibility of the parent/guardian/student. 

• Student Code of Conduct will be enforced.

• Lost/Stolen Device - Student/parent/guardian should immediately notify building administration immediately

• Lost - Cost is the responsibility of the parent/guardian/student.

• Stolen—Devices reported as stolen outside of School require that parents notify the police and provide a copy of an official police report to the school administration. Theft Inside School requires a letter signed by the parent/guardian stating the date, time, location, and description of the circumstances.

Repair and Replacement Fee Table:

Do not replace lost or damaged chargers on your own.  Unapproved chargers will void the warranty of the Chromebook and Chrome tablet.

By agreeing to the terms of this agreement, the student is given the privilege to use the One2One device at school and at home. If the student and parents/guardians disagree with the terms of this agreement, the student will be limited to the use of the device at school only.

Parent/Guardian: I have read and understand this agreement and give permission for the school to allow my child to use the Borden-Henryville School Corporation One2One device under the terms and conditions set forth above. Your student WILL NOT be able to receive their device until this paperwork has been returned to their school. I have also made a selection to accept or reject the Accidental Damage Coverage option listed below.

Accidental Damage Coverage

By checking the appropriate box below, you are accepting or rejecting the option to purchase the $30 Accidental Damage Coverage. See Student Device Policy 7440.03 for Accidental Damage Coverage Details. 

  I ACCEPT the option to purchase the $30 Accidental Damage Coverage through BHSC for the student listed above. (Payment will be made today OR dropped off to your student’s school office OR mailed to your student’s school within 2 weeks.) 

  I REJECT the option to purchase the $30 Accidental Damage Coverage through BHSC for the student listed above. (I reserve the option to accept and pay for this coverage no later than 2 weeks from the date of device issue, but my student’s device will require an inspection before Accidental Damage Coverage can be added. I understand I will be responsible for any and all damages that occur during or after this 2-week time period.)

Payment: Payment is due within 2 weeks of your student receiving their device. Checks are preferred and can be made payable to your student’s school.  This form will need to be mailed or turned into the Office at your child’s school with payment attached.  The $30 payment CANNOT be made through Infinite Campus.
